Sign-in/Waiver Sheet - This form is 2 pages long.  Please print both pages (sides) if you are going to use it and will have more than 20 participants (updated 10/2023). NOTE THIS SIGN IN SHEET DOES NOT INCLUDE THE INCIDENT REPORT FORM, NOW A SEPARATE DOCUMENT.

Incident Report Form -  Leaders should carry a copy whenever they lead an event. If someone is injured at a TriState Ramblers event and had, or is likely to require, medical treatment at a medical facility, complete an Incident Report form (updated 2/2024)
